I'm wondering if you'd ever see a prototype where an engine had two or three cars in front of, and behind the engine, while doing some industry switching. I can't see why not, other than lousy visibility, but that seems to apply anytime one pushes cars.

Yes

Definitely.  But they engine would have to protect movement in either direction by having a crew member ride the leading car in the direction of movement over crossings or locations where there were switches.

As Dave said,

Yes. I've done this a few times before. You basically need a brakeman and conductor on duty with the engineer, as one will ride each end of the cut. It's cumbersome, but sometimes it's the only way to make progress. You try to minimize the time that the loco is sandwiched, though.
